A 9-year-old NM DSH with a history of lethargy and poor appetite was presented for evaluation of pyrexia that was non-responsive to antibiotics.

Image Interpretation

The liver is of normal size and echogenicity. The spleen is enlarged and has a miliary pattern of hypoechoic micronodules. The spleen and liver were were aspirated.

DX

Granulomatous hepatitis and splenitis due to histoplasmosis.
