Splenic and hepatic infiltrative pattern with splenic nodules. 25-gauge ultrasound-guided FNA of the liver and spleen were performed without complications. Suspect lymphoma or multiple myeloma. Mast cell disease is also possible versus splenitis and reactive hepatopathy, which is less likely.
The spleen was significantly enlarged and measured 2.0 cm in width with mixed hyperechoic and hypoechoic nodular changes. A slight amount of free fluid was noted adjacent to the spleen.
The liver was uniformly hypoechoic to falciform fat, irregular and enlarged.

Renal disease – chronic kidney disease, obstructive uropathy, pyelonephritis
Cytology of the liver and spleen revealed plasma cell sarcoma/myeloma of the liver and spleen.
