An 8-year-old FS German Wirehaired Pointer with history of OHE and normal CBC and blood chemistry two months prior was presented for vomiting and anorexia. On physical examination dehydration and a tense abdomen on palpation was present.

Sonographic Differential Diagnosis

Kidney: Cortical infarction possible owing to pyelonephrtitis. Primary renal neoplasia is suspected however owing to the focal loss of structure. Aggressive pyelonephritis with secondary infarction possible.

Image Interpretation

The left kidney revealed irregular cranial pole and dorsal cortex with loss of corticomedullary detail and stricture. Pyelectasia was also present. reactive surrounding o,fat inflammation was present. Power Doppler of the affected region was void of blood flow.

DX

Renal adenocarcinoma. Mammary gland: cystic hypertrophy of interlobular ducts.

Outcome

The patient was recommended for immediate emergency exploratory surgery at referral facility. Exploratory surgery was performed which included a right nephrectomy, mammary gland removal, along with biopsies. Post-operatively the patient did very well. Follow-up blood work was within normal limits. An oncology consultation/chemotherapy was discussed with the owners, which was declined.

Clinical Differential Diagnosis

Liver/renal/pancreatic disease, peritonitis, surgical adhesions, neoplasia, torsion of intestine/stomach/spleen, trauma.

Sampling

Full-thickness surgical biopsies of the right kidney revealed renal tubular adenocarcinoma and biopsy of the mammary gland revealed cystic hypertrophy of interlobular ducts.
