An 8-year-old NM DMH was presented for evaluation of tachypnea and a cat bite abscess on the forehead.  Thoracic auscultation was normal. Thoracic radiographs revealed loss of detail in the diaphragmatic region between heart and liver with the presence of a soft tissue opacity in the mid region in both views.

Sonographic Differential Diagnosis

Pericardial diaphragmatic hernia of liver and falciform fat. Congenital lesion with likely recent clinical signs owing to hernia movement-sliding hernia.

Image Interpretation

Pericardial diaphragmatic hernia of the liver and falciform fat. Liver touching the heart.
