  • Abdominal ultrassonography because an inguinal hernia
  • Found bilateral adrenal masses with invasion of the caudal vena cava
  • Nodule in the spleen and prostate with prostatomegaly
  • Pheochromocytoma / Adenocarcinoma?
  • What other differentials do I need to keep in mind?

Video one nice left adrenal

Video one nice left adrenal from the right side using the cvc as a window. Be sure to fna that splenic nodule because pheos love to met to spleen in my experience. Im not seeing bilateral masses but a left adrenal mass invading the cvc. if looking cushingoid then adenoca but if not, and BP is > 180+ systolic think pheo.  Marshfield lab does catecholamine urine test. if pheo it will be very high. Or can fna 25 gauge but watch adrenaline surge. Just happened to me once. Invasive masses are eithe rpheo or adenoca by definition. Nice images.

It actually has clinical manifestations of cushing's syndrome and the BP is < 180.
