Adrenal Mass As Cause Of This Dog’s Collapse???

Brutus, 13 year old Rott. Cross.

Started with respiratory distress and labored breathing today.

Labs: Creat-3.9  BUN-58  Phos-8.0  K-6.1

Chest Rads: Some increased interstitial pattern in cd/dorsal and middle lungs. Heart shadow looks smaller than normal

Quick Cardiac U/S: seems like LV walls greatly thickened but with smaller size of LVID I’m leaning towards under/poor circuatory perfusion

Abd U/S: normal except for what I think is an adrenal mass on left side. Couldn’t really see Rt adrenal but I included a loop in case I’m missing something.

So I’m grasping at straws, no cardiac disease, no pleural disease, no splenic mass or bleed out…Maybe pneumonia, but some sort of neoplasia is high on my list…

And by the way…about 5 months ago this dog had a bout of epistaxis which did resolve but never got a clear Dx of why.

We did check BP’s which were normal.

Looks very pheochromocytoma

Looks very pheochromocytoma to me and collapse fits as well as nose bleeds and hypertension. You can run a urine catecholamine for Marshfield labs to confirm.

Looks resectable but CT best for that or approach the left adrenal form the right imaging through the cvc tio ensure no cvc  invasion like this one attached. For this maneuver get a clean right adrenal from SDEP 14 position keep manual pressure to minimize the real estate and slide caudally til the left adrenal mass comes into view in the far field with the CVC in the near field.

if you get that view then please post the still image similar to mine attached here. Would be a cool ending to the thread.

Sam- can you also post a couple of cines from your right evaluation. I have found that EL can see them and now that I am slowing them down I am able to find many of them on my own.
