I have a question about the SDEP protocol. When you view the R kidney through the intercostal space to get it horizontal- how do you get that right adrenal in view when you really can’t get under that kidney. I hope my question makes sense. Not much mobility there. When I go caudal I get that tipped view of the R kidney and of course the adrenal is often a bit more difficult to see.

yes Randy you are correct adrenals are always much more complicated.. good point. Just got this latest info form Dr Obrien and thought to share it. Research and measurment normals are wjat they are but always on a small population… there are always those cases of nodular enlarged adrenals that are benign and even myelolipoma or other masses or big adrenal cases that even have addisons..

So its important to look at the adrenals but look at the case… and sometimes just forget about what th eadrenals look like and treat the dog:)

 

Regarding the right kidney right adrenal its important to point dorsally to the spine and then drop the probe tail and push to ensure there is no space between the body wall and right kidney before you start to drop the tail of the probe to bring in the right adrenal. If you have any structure like colon or small intestine between the body wall and RK you will miss the adrenal. In big dogs you can flip them to left side down and approach the RK from the paralumbar. If you are at ACVIM June 4/5 in Indiannapolis come by booth 231 and I will show you as we will have a live dog and scanning demo at 1 and 4 pm every day and also a stuffed dog to show positions so our french bulldog wont get overscanned. We have a Phantom as well to show USG fna procedures as well.
