-10 year old FS
-diagnosed with hyperthyroidism and positive urine culture 8/16/14, no murmur or arrythmia at that time
– thyroid controlled now, although weight has dropped from 6.2 to 5.7 lbs
– urinary tract symptoms resolved after 30 days antibiotic but re-occured 11/13/14, bacteria again reported in urine and culture is pending

– at this time she was also found to have murmur and arrhythmia. Cardiopet: a regular supraventricular rhythm is present. A P wave is noted preceeding each QRS complex, but with a long PR interval at 175 msec. Two differentials for this ECG are possible. The first would be first degree A-V block and the second would be isorhythmic AV dissociation. In either case, this could be secondary to underlying cardiac disease or potentially fibrosis of the AV node or secondary to age-related changes. Contribution of high vagal tone could also be present.

The rhythm and heart rate does not look abnormal to me on the cine loop.
I can’t see the IVS real well on my screen (to dark). The IVS does look a bit thickened compared to the VFW. Should be less than 6 mm in diastole. It would not surprise me if they were thickened because this cat is hyperthyroid. On the transverse image the R side of the heart may be a bit dilated.
The L atrium may be a bit enlarged- but not grossly so. I measured LA/AO on 2D around 1.6
I don’t think I see SAM – CFI might be helpful.
If this cat is losing weight I would look elsewhere.

Arrhythmagenic for sure but heart looks pretty normal in these views. Thicknesses, chamber and functions are fine and drops a bit of fs% when the arrhythmia kicks in. Look for infectious or other systemic disease here.
When arryhtmia kick in or pauses then the la and lv volumes will increase artificially and i think the LA is doing that here but not an issue as it normalizes in sinus rhythm

Pam at 3 oclock thats just normal fat at the heart base. Im not appreciating anything structurally abnormal in that image and its obliqued & oblique images can make pathology artificially.
