Bradyarrhythmia in a 9 year old MN German Shepherd Dog with syncopal episodes

A 9 year old neutered male German shepherd was presented for syncope. Physical examination revealed bradycardia as the only abnormality. On ECG, the heart rate was 45 bpm and atrial flutter with a 3rd degree AV block was present. ACTH stimulation test was normal.

Sonographic Differential Diagnosis

Bradyarrhythmia, but good cardiac function. Third degree AV block.

Image Interpretation

Mildly excessive left ventricular outflow velocity was noted in this patient, which is consistent with compensatory hypercontractility noted in the left ventricle. A persistent bradyarrhythmia was noted in this patient, consistent with the third degree AV block that was found on electrocardiogram. Note how the right atrium contracts twice as fast as the left atrium. A pace maker would be indicated.

DX

Bradyarrhythmia, good cardiac function

Outcome

The patient was referred for a pacemaker.

Clinical Differential Diagnosis

Primary conduction abnormality, AV block secondary to cardiac disease (myocarditis/trauma/anoxia), electrolyte changes, acid-base imbalance.
