Sonographic Differential Diagnosis

Passive congestion of the liver secondary to right-sided heart failure.

Image Interpretation

Generalized hepatomegaly with dilated hepatic veins (large arrow) consistent with a passive congestion is seen. The vena cava is also prominent. Anechoic pleural effusion (thin arrow left of the diaphragm) also can be seen (best seen in video 2.)

DX

Generalized hepatomegaly, passive hepatic congestion due to right-sided heart failure

Comments

This case was included to demonstrate an example of passive hepatic congestion even though only a minimal database was obtained.
