Renal mass in a 13 yr old FS mixed breed dog

• 13 year old FS mixed breed dog (looks like boxer/pitbull) presented for recurrent SQ mass on the front leg, anterior antebrachium.
• This dog had a low grade soft tissue subcutaneous sarcoma narrowly excised from the same site a few years ago.
• Primary vet noticed increased soft tissue opacity in the retroperitoneal space with mass affect displacing colon and all viscera ventrally on abdominal radiographs

• 13 year old FS mixed breed dog (looks like boxer/pitbull) presented for recurrent SQ mass on the front leg, anterior antebrachium.
• This dog had a low grade soft tissue subcutaneous sarcoma narrowly excised from the same site a few years ago.
• Primary vet noticed increased soft tissue opacity in the retroperitoneal space with mass affect displacing colon and all viscera ventrally on abdominal radiographs
• Abdominal ultrasound showed a 2.5-3.5cm complex hypoechoic mass that appears to be originating from the capsule of the right kidney, medial side of the caudal pole.
• US guided FNA was nondiagonstic: “Peripheral blood and rare small sheets of benign mesothelial cells without evidence of inflammation or neoplasia. The cellularity of the sample is not sufficient for a diagnosis.”
• I know the next step would be core biopsy although I am leery of hemorrhage owing to its location and potential high vascularity.
• Would love to hear your differentials for this lesion…thank you!

IMO, I would put

IMO, I would put hemangiosarcoma at the top of the list as it appears to be expanding out of the caudal RK and into the retroperitoneal space which HSA does as opposed to nodular neoplasmas such as lymphoma which may distort the renal capsule but tend to stay within the capsule. The vascularity may also support HSA. Removing the kidney if not evidence of mets elsewhere is likely indicated.

Also, HSA is notoriously hard to diagnose with FNA as opposed to round cell neoplasms.

Thank you.  I usually get good returns on my FNA’s so this helps me to feel better :).  I offered a core bx next but not sure if i want to do this with a renal HSA.  There is a suspected recurrent SQ sarcoma mass on the front leg of this dog so I am worried about metastases.

Renal HSA til proven otherwise dont damage the capsule with fna if no other lesions just get it out of there ive had these cases go on long term with just nephrectomy.
