Haemorrhagic pleural effusion

  • 12 yo MN GSD, mildly anaemic and tachypnoeic, no history of cough
  • Pleural effusion appeared haemorrhagic when tapped
  • What is mass type lesion at heart base and on transdiaphragmatic view on ultrasound?
  • Xray is also unusual, any insight?

Hello!

The mass effect by the heart base looks like it has possible air entrapment in it. The primary differentials for a geriatric dog with hemorrhagic pleural effusion are neoplasia and lung lobe torsion, with neoplasia the primary rule out.

Cytospin / cytology of the sample of pleural effusion would be suggested. I would also recommend checking the spleen for evidence of neoplasia.

A coag panel would also be recommended just in case its a coagulopathy.

The radiographs indicate pleural effusion, obscured cardiac silhouette and possible pulmonary pathology.

The dorsocaudal soft tissue

The dorsocaudal soft tissue opacity on xrays was interesting, no DV unfortunately.

I suggested fluid cytology but the dog was PTS due to suspected neoplasia.

Clotting was normal.

I am just hoping to learn from these cases.
