– 6 month old F minature Australian Shephard presented for pre-anesthetic evaluation for spay wt 10.9kg
– grade 4/6 systolic heart murmur heard on both sides of the chest; clinically well
– I believe there is a VSD? with no significant cardiac remodelling
– there is also AI present but I think it may be due to the position ofd the VSD?
– how closely should this one be monitored?

Loud VSD? Good thing… the
Loud VSD? Good thing… the grade drops over time??/ bad thing… it means the hole is getting bigger and right sided pressures are increasing. So tincture of time here to see where it goes. I think you are offline in your spectral doppler as you should be getting a solid left to right velocity of > 5 m/sec. The third video I would put the color at 9 oclock instead of 3 oclock position as I can see the VSD at 9-10 oclock popping in and out.
At 6 months he still has some growing to do will have to see at 9-12 months of age on a recheck on whether its going to be hemodynamically significant or not but its a decent sized one moprphologically so there is some reason for concern in my opinion and you may start to see some compensatory hypertrophy at that time.
