13 yr old FS DSH with echogenic left liver mass

13 year old FS DSH with history of vomiting.  CBC is wnl.  Chem prof shows an elevated ALT.  Abdominal US shows a diffusely thickened small intestinal muscularis layer, an echogenic left liver mass, irregular pancreatic body enlargement, slightly thickened gallbladder wall and gravity dependent echogenic sludge.  

I don’t frequently see echogenic feline liver masses and am curious iif this is related to the  GI pathology (metastatic disease) or just an incidental finding (benign tumor).  FNA’s were declined :(.

What is is on your differential diagnosis list for an echogenic liver mass in a senior cat?

Hepatocellular carcinoma

Hepatocellular carcinoma possible as cats dont do liver nodules well but on occasional fatty nodules occur in older cats. You can track it to see if growing which increases the likelihood of malignancy oif they wont allow fna

couldit be an incidental

couldit be an incidental event – granuloma etc?  doesn’t look like a metastasized tumor…or unusally hyperechoic nodular hyperplasia or abscess ( no concrete evidence thereof ie, gas pocket etc.  or could it be part triaditis causing pyogranuloma!  just a thought!

Good thoughts for sure….

Good thoughts for sure…. but I would lean against granuloma or abscess because these both typically have ill defined inflammation around them (not prsent here) and in this lesion you can see its well-defined on the borders and passive on the architecture… like snow on grass as opposed to a hole in the grass if that makes sense.

The owner declined fna’s but

The owner declined fna’s but the cat is doing well on pred.  I think it was also placed on anitibiotics as well.
