Vegetative lesion, heart worms


Dear All,

This is a 6 years old male hunting dog. He was found Dirfilaria immitis positive with ELISA Ag. Test. 

Also severe heart murmur is present.

The dog is coughing, and has exercise intolarence.

Thorax x ray pending. Blood work pending.

US cardio: I can see 5-6 adult heart worms in the pulmonary artery, TR is present 3 m/s, mild MR is present, no pulm. insuff.

I am worried about the lesion in the semilunar valve of the aorta. Looks like a vegetative lesion to me- turbulence is present. Ao velocity systole: 3 m/s, ao insuff dyastole: 2,5 m/s

Would You recommend an adulticid  ( melarsomine) theraphy in this case? Would You treat the aorta insuff? Should we think about hemoculture?

    I would think basket retrieval if possible but slow kill may do it as well… I know that no parasites grow in austria but let me see if Peter has input here:)

    The owners would do for the dog everything.
    I am worried about the leaflet of the aorta.
    Can it be a congenital defect? Or an old infection?
    Can this kind of lesion cause exercise intolarence? Or the clinical signs exisist only because of the heart worms?

    Hi Rita – are you able to send an image that shows the heartworms? I’ve not seen this before and would like to be able to recognize them.
