Thickened stomach wall

Hi all,

I just returned late Sun from my first SDEP Abd scanning lab (Loved it!)  and Monday morning brought one of my dogs in to US.  I was evaluating his liver (Alk Phos up for a few months, and TP trending low normal), but ended up focusing on his stomach.

Hi all,

I just returned late Sun from my first SDEP Abd scanning lab (Loved it!)  and Monday morning brought one of my dogs in to US.  I was evaluating his liver (Alk Phos up for a few months, and TP trending low normal), but ended up focusing on his stomach.

  • 8 year old MN Terrier X (“rescue” from euth for skin problems dog)
  • Bout of severe vomiting, progressing to fresh blood in vomit
  • On Apoquel and Thyroxine
  • Monday BW:  TP = 4.6 (5.5-7.5), both Alb and Glob equally low
  • Hct 43%

That stomach wall!  1.1cm and 0.8cm, diffusely thickened, and darn if I can see layers?  And fuzzy fat all around it.

I’m thinking of sticking a needle in it…. but being my first US on return from SDEP, am I jumping the gun ??

Thanks,

Karen Ebersole

I agree with your

I agree with your interpretation Karen… transmural disease with ill defined fat and loss of layers. Bad gastritis can do this. A small epigastric LN is up as well in video 1. Im actually thinking more ulcer here see my annotated version of your still image because this seems mostly mucosal and the muscularis and serosa are in tact. This is a tough one to get definitive fna results as opposed to a solid lymphoma pattern. I suggest scope or explore as you may have to sx correct an ulcer.

Thank you Dr. Lindquist!  I

Thank you Dr. Lindquist!  I hadn’t spotted the intact muscularis and serosa.  That’s good to see.

I will likely sx explore.  We don’t have a scope;  I prefer the US for sure.

An update…. “Jack” was

An update…. “Jack” was acting near normal within a few days; and a quick US 5 days after the first one showed a near normal stomach wall thickness and layering.  Wow.

Repeat BW:

  • Hct back up to 50%
  • TP low normal, 5.2?

I think something is still brewing in there…. but he’s acting so good currently, I’m reluctant to do exploratory sx.  We do have a mobile surgeon who also does endoscopy, so he’s an option too.

I’ll post further updates as I go.
