Splenic Lesion

Libby is a 10 year old Cavalier King Charles Spaniel that was in for dental work today.

I often place the probe on the abdomen just for practice.

While I was taking a quick look at Libby- I noticed a cystic like lesion on the head of the spleen. 

I would like some feedback- should I be worried? List of differentials. 

I would also like to wish all members of the Sonopath Community a very Merry Christmas and Happy New year!

It has some architecture to

It has some architecture to it but most is lost and is dramatically hypoecheoic. Needs a needle at least if not removal altogether. Diffs: nodular hyperplasia, abscess, cyst, hsa and round cell neoplasia. If worst case HSA is in play the only ones I ever see back for recheck are the ones I found when there fore something else. Once they are clinical for splenic HSA then it has usually spread already in my experience or get ready for some heavy chemo.

May also be an incidental

May also be an incidental lesion so another approach would be re-scan in 4 weeks and if the lesion is growing then intervention is needed.

I have been a bit “gun-shy” since I had that fatal bleed post aspirate on a dog about

a year ago.

25 g, sedateany animal

25 g, sedateany animal minimally tense or jumpy, benadryl if suspecting mct… never had a problem with those precautions…. ears pinned back and not sedating or MCT are the only time ive ever had complications… the tense dog wihtout sedation the spleen stays in position and the body wall tenses and moves the need so can lacerated the spleena dn you dont see anythign necessarily at first but then bleed after. Or VWB can do this too.
