2 yr MN Shepherd mix, previously healthy until one week ago when became anorexic and lethergic. Unclear if may have previously had any PU/PD symptoms. Current on vaccines including two years of annual Lepto vaccine. Blood work = creat 12, BUN 238, Phos 24, TP 9, WBC 19,000, HCT 31.8. UA = SpG 1.015, occasional granular casts, otherwise inactive. Lepto PCR on urine pending. Concerns on ultrasound – severe loss C-M distinction, left kidney significantly smaller than right. Congenital renal dysplasia at top of differential list now. Thoughts?

Renal dysplasia possible but
Renal dysplasia possible but consider chronic lepto or other causes of chronic interstitial nephrosis as this is the primary pattern here with thick uniiformly echogenic cortices that can disrupt the pelvis and cm junction owing to chronic inflammation and fibrosis… needs renal bx to know but doubt you can do anything here to help the pet these kidneys are chronically ugly unfortunately.
