Paralumbar abscess

  • 3 year old dog spayed 5 weeks prior presented initially with back pain and then swelling over back.
  • Large abscess drained over lumbar muscles, no bite wounds or FB seen. Scan requested to assess for abdominal involvement.
  • Left retroperitoneal area was inflammed and oedematous with fluid lines. The caudal pole of the left kidney appeared infarcted. There was an anechoic area with central shadowing hyperechoic structure in region of both ovaries – suspect sutures from neutering? A severe sublumbar lymphadenopathy was present. No swab seen.

    • Could this all be a complication of spay? I would not expect to see retroperitoneal inflammation >5 weeks post spay…
    • Other hypothesis is abscess from injection site reaction or tracking FB – would you see retroperitoneal inflammation associated with adjacent abscess even if external to abdominal wall?

its right in the area of the

its right in the area of the ovarian pedicle… maybe suture reaction… one of my first cool finds as a sonogrpaher was in 1997 where there was an asbcess in this area and penetrating the adjacent body wall and i pulled out a plastic garbage tie that some old school vet used 10 years earlier to tie off the o pedicle lol… I was able to fish it out with an alligator forcept us guided…but a gut reaction or similar possible here I would explore and clean up that area surgically.
