Nephritis ?


Hello,

This is an 1.5 years old large Rottweiler MN ( neutered at 6 months). He is vomiting and anorexic. Bloodwork showed established inflamation with moderate  increase in BUN and Creatinine. USG was 1.015. X-rays were wnl. UA pending

AUS : hyperechoic thin cortex with disrupted architecture on both kidneys.

Despite being neutered as a puppy prostate appears to be symetrical enlarged. MILN was approx 0.8 cm

I have 2 questions :

I have 2 questions :

1. Can this 2 findings ( prostate and kidneys) being related? Can this be neoplastic ? like lymphoma even though the MILN is under 1 cm and maintain length to height ratio?

2.My DDX for kidneys findings are Nephritis/Ethylene Glycol/ Nephrosclerosis/ Neoplasia. I suspect an acute process. Can this be lepto? Any thoughts?

    Wow, renal and prostatic lsa certainly is primary on the list. Dr Ondreka’s team is doing a study exectly on this at U of Giessen. I would fna the prostate and kidney and see. If you get a + hit for lsa please email us at info@sonopath.com and we may be able to include this in the study.

    Thank you EL. Can this kidney presentation be toxin insult ? Like ethylen glycol?Not entirely sure if prostate or retroperitoneal fat in the region of pancreas. I did recommend FNA of the kidneys. Will clarify at FNA time.

    Good thought jacquie on renal dysplasia with secondary degenerative… toxin possible but what concerns me here is the complete disruption of curvinear detail of the cm junction and rest of the kidney … which is a main neoplastic criteria… see examples of ethylene glycol, renal dysplasia, and renal round cell neoplasia here respectively from the sonopath search archive:

    From my understanding the majority of renal dysplasia comes with small kidneys but not always.
    Will atempt FNA to rule out LSA.

    Was vaccinated for Lepto, normal liver values. At the time ofUS lepto test was pending.

     Will try to find out;if positive I’ll update outcome
