Mass medial to shoulder

  • 11 yr old MN DSH
  • 1 week hx of acute onset LF lameness
  • Round swelling medial to shoulder, apx. 3″ diameter
  • No response to Onsior

I saw this cat as a second opinion today.  His leg palpates WNL, the leg rotates laterally because of the pressure of the swelling/mass.  The O wonders if he fell from his usual high hiding place.  Referral rads show no orthopedic abnormalities in the elbow and shoulder.  I can’t see much of the soft tissue in the axilla on the rads.

I put the US directly on the mass, with the cat in right lateral recumbency and the LF leg pulled caudally.  Doppler wasn’t working well on our machine today, but my ddx are mass, hematoma, or abcess.

Do you think there is a pocket of fluid in there?  I scheduled him to come back for a FNA while under sedation.  I don’t normally look in the axilla/shoulder… but thought it was as good a try as anything else.  Thank you!

Looks like a cystic mass – needs a FNA of both the fluid and of the more solid tissue.

Looks like undifferenitated sarcoma there. I agree needs a 25g fna of the parenchymal portion and fluid for culture in the mild potential for septic lesion.
