Liver Mass

  • 12 year old Collie MN dog with lethargy, intermittent vomiting and elevated liver ezymes
  • Scan showed bilateral adrenomegaly, mineralisations in spleen, heterogenous pancreas, cystic/sludge gallbladder, hyperechoic patchy liver with hyperechoic mass in right side of liver
  • FNAs taken of mass and cytology reports: hepatocyte cytoplasmic rarefaction (moderate), lipofucinosis (mild).
  • What do you think this mass is and should it be biopsied?

Unfortunately, the cytology

Unfortunately, the cytology is not really telling us much about the types of cells in the mass, whoch may be a good thing..at least they did see evidence of neoplasia. I think lipofucinosis is a “wear and tear” finding woth cells and the rarefaction, I think, is associated with cellular lipid but may also be seen with inflammation and storage diseases. 

Its hard to say what the liver mass is as far as benign vs aggressive. I think a biopsy of the mass is needed for further diagnosis. 

Any signs of Cushings? Mineralized splenic nodules have been associated with Cushings as well as the adrenomegaly.

I don;t like the disruption

I don;t like the disruption of architecture in the mass and Im thinking carcinoma may be in there somewhere (fish in a pond thing) but is often a luck of the stick position. Its not resectable in that right cranial oposition and may be very low grade but I would followup in 3-4 weeks with measurments and check doppler for vascularity. Bx technically yes but these things are mushy poor integrity and tend to ooze so get one good stick with 18g right intercostal SDEP 12-13 positions.. I wouldnt go big here on gauge.
