Hydronephrosis in a different cat

  • 14 year old FN DSH presented unwell and inappetant with progressing severe azotaemia, is producing urine
  • Scan showed small R kidney with cortical infarcts and poor blood supply. L kidney hydronephrotic with dilated ureter which I could follow distally but then would lose it. There was an area of mineralisation/drop out but I was not convinced this was intraluminal and could be colon, what do you think of this structure?
  • Is it unusual to have a ureteral stone if there are no other stones in kidneys or bladder?

    • What is cranial to right kidney? Adrenal/LN?
    • This cat also had an enlarged pancreas esp LL
    • I am wondering if this cat would be suitable for a SUB…

This looks like a ureteral

This looks like a ureteral stone and a tortuous ureter likely owing to chronicity. Hydro from scarring or that small mineralization in the renal pelvis. Needs an SUB device. Stone movers have their stones anywhere at any given time… obstruct, infarct, hydro, scar…what you are seeing here is the results of periodic insults in this cats life along those lines.

Nice job finding the ureterolith.

I have in the past actually applied a dental scaler over the stone to break it down more (gently). It can cause scarring but that may already be a fate accomplie from the initial wedging of the stone. Just an idea. Worked for me when the owners couldn’t afford referal.

Great idea you should publish that technique! Chick and Allyson wouldn’t be able to charge half what they do at AMC for SUB and stents lol.

You have to be careful about

You have to be careful about heat so you apply very short bursts over the stone and from difevent angles and positions.
