My question is about the gall bladder. I see some typical gravity dependent sludge but also what looks more like soft tissue. I tried looking at it with color flow but the guy was just too wiggly (as you’ll see in the rather poor-quality clips I have). For now, we are going to follow up and have bile acids pending but just wanted to get some opinions on what you think. I did turn him over to see if any of it would settle but it didn’t.
The still image I’m posting is with the animal standing and imaging from underneath. The clips are of him in dorsal recumbency.

This is likely an attached
This is likely an attached bile clot with some stones in it. I see this in the apex a lot. You can put power doppler over these to see if vascular which would define it as tissue and zoom in hi resolution and usually see a clear separation of the GB wall surface and the bile. When in doubt rescan in a couple of weeks to be sure its not a biliary carcinoma but I doubt it would be a very odd pattern and if you look at this one the Gb wall is solidly in tact and not deviated from its normal contour.

Is it worth trying ursodiol on bile that looks like this?
I will be following up on the splenic mass anyway so will likely just rescan down the road.

Hmm debatable on the ursodiol… its passive sludge not obstructing, GB not overtly dilated… would be interesting if it reduces under urso but I can’t say its “necessary”…. yet:)
