FIP?

– severely cachectic cat age unknown but likely a senior, MN Devon Rex x

– bloodwork moderate anemia, neutrophilia, T4 and biochem wnl; fecal test negative for parasites

– profuse, foul smelling diarrhea, no vomiting and eating Recovery moist diet

– u/s: trace effusion, bilaterally enlarged (5cm) kidneys with hyperechoic cortices, enlarged jejunal LN’s

– rest of scan unremarkable – GI regular layers, maybe submocosal and muscularis layer a little thicker than normal?

– FNA of jejunal lymph nodes: pyogranulomatous inflammation

Older cats get the dry form of FIP and in the archive I have a few of chronic triad cats that were actually FIP dry form once full thickness bx were obtained btu an unpopular procedure so tough to knwo how much is actually out there. Dry old cat FIP I believe is underdiagnosed. That being said underlying chronic resistant bascteria-lymhpadenitis can do this as well. The minor free fluid localized around the LN comes from hydrostatic pressure from the lymphadenopathy-lymphatic strangulation and will pool around the mesenteric ln like in this case first. You can culture fna samples from these nodes and get meth-resistent bacteria and such. Dry fip always a concern with pyogran samples of course.

In Europe there’s one lab

In Europe there’s one lab where they have recently started to identify mutated Corona virus by PCR of aspiration specimens. This might the best way to rule in /out FIP in these patients. 

Maybe this is possible in your country as well??
