A 15 year old NM DSH was presented for losing weight with a history of a possible abdominal tumor and kidney stone. Both CBC and serum biochemistry were within normal limits.

DX

Renal carcinoma

Sonographic Differential Diagnosis

Left renal mass. This is suggestive for lymphoma pattern or similar neoplasia. Fine-needle aspirates would be warranted and should be submitted for cytology review.

Image Interpretation

The left kidney in this patient had a 6.0 cm mass with focal areas of corticomedullary calculi and pyelectasia. Subcapsular hypoechoic tissue was noted. This is highly suggestive of lymphoma. The right kidney measured 3.9 cm with corticomedullary calculi. Slight irregular contour was noted. This may represent an early lymphoma pattern. However, this is not overtly suspected.

Clinical Differential Diagnosis

Abdominal mass – neoplasia, granuloma, or abscess of an internal organ. Renal pathology – hydronephrosis, pyelonephritis, renolith.

Sampling

Ultrasound-guided fine needle aspirate of perirenal area. Cytology revealed carcinoma with necrosis.
