RAD – Obstructive gastric foreign material in the gastric outlet of a 6 year old FS Pit Bull dog

This 6 year old FS Pit Bull dog presented with restlessness of 2 hours duration. Owners concerned about obstruction, has access to many potential foreign bodies.

Image Interpretation

Rads of thorax and abdomen- Osseous structures:
There is a degenerative lumbosacral stenosis including bridging spondylosis at L7/S1,
vertebral endplate sclerosis and spondylarthritis.
Intrathoracic structures:
The mediastinum and pleural space are within normal limits. The esophagus contains a
moderate amount of gas on the left lateral view. The lung reveals a mild generalized
increase in interstitial opacity emphhasizing the caudal lobes which is compatible with
an age related incidental finding.
The cardiac silhouette is within normal limits for size and shape. The major vessels are
within normal limits. The pulmonary vasculature is thin consistent with hypovolemia.
Intraabdominal structures:
The serosal detail is within normal limits. The liver margins extend beyond the costal
arch but are pointed. The spleen and kidneys are within normal limits for size, shape
and position. The urinary bladder is moderately distended. The stomach is moderately
distended wiht food and gas. The pyloric antrum and gastric outlet region are distended
and contain streaky soft tissue opaque material which is surrounded by gas and
consistent in position throughout all views. The small intestinal loops are in their anticipated position and non dilated. The diameter of the small intestinal loops is sven
throughout the entire population. The small intestine contains mainly gas and reveals a
maldigestion pattern. The colon contains gas and a moderate amount of fecal matter.

DX

The radiographic findings raise the suspicion of obstructive gastric foreign material within the region of the gastric outlet.

Outcome

The radiographic pattern is compatible with
fabric. There is some overlap between the radiographic appearance of textile material
and gristle or other organic matter wich is referenced here. Oral intake if gristle or
similar should be ruled out.
Abdominal ultrasound is indicated to confirm the presence and position of the possible
foreign material prior to any further measures, to rule out additional foreign material
and to rule out behaviour of the material as linear foreign body.
Depending on the findings endoscopic or surgical removal may be considered.
