RAD – Bilateral chronic degenerative osteoarthosis of the talocrural joints in a 10 year old FS Maltese

This 10 year old FS Maltese has a history of intermittent lameness of one year duration, seems to be worsening recently.

Physical exam: bilateral swollen hocks, left is worse.

Image Interpretation

Pelvis: Moderate generalized decrease in volume is noted for the left musculature. A small osteophyte is noted at the left formal head.

Right and Left hind limbs:  The periarticular bones of the talocrural joint present marked osteophyte new bone formation emphasizing the medial malleolus with associated bulging of the overlying medial soft tissues. At the plantar aspect of the tarsal joint marked enthesiophyte formation is noted bridging the talocalcaneoquartal joint. The medial aspect of the talocrural joint presents moderate widening with deformation of the juxtaposed joint surfaces with subchondral bone modeling. The medial trochlear ridge of the talus presents flattened and presents a mild irregular surface.

 

DX

The findings are consistent with chronic degenerative osteoarthrosis of the talocrural joints bilaterally

Outcome

Even though osteochondrosis is a rare entity in toy breeds the
findings of the right talocrural joint are suspicious for osteochondrosis as underlying
disease. Ultrasound-aided synovial aspiration should be performed to rule out
immune-mediated disease.
