Prostatic transitional cell carcinoma diagnosed by FNA in a 15 year old MN dog.

A 15-year-old MN dog was presented for persistent hematuria. On physical examination a cardiac murmur was present. The only abnormality on urinalysis was an inappropriate specific gravity. Elevated ALP activity was present on serum biochemistry. LDDST and blood pressure were both within normal limits.

Sonographic Differential Diagnosis

This is highly suggestive for transitional cell carcinoma.

Image Interpretation

The bladder in this patient presented a 4 x 2.4 cm luminal bladder mass with mineralization that occupied the caudal aspect of the bladder and invaded the cystourethral junction and the urethra. Significant power Doppler signal was noted, indicative of high vascularity.

Prostatic carcinoma

Comments

This is non-resectable. Poor long term prognosis. Stent placement, palliative treatment with COX-2 inhinitors or alternative therapy or chemo-embolization are options.

Clinical Differential Diagnosis

Persistent hematuria – bladder pathology (neoplasia, cystitis, uroliths), prostatic pathology (neoplasia, infection), urethral pathology (neoplasia, urethritis). Murmur – mitral/tricuspid insufficiency.

Sampling

FNA of prostate. Consistent with prostatic carcinoma, necrosis and chronic hemorrhage.
