Polycystic kidney and liver disease in an 11 year old FS cat

An 11-year-old spayed female cat, of unreported breed, was presented for evaluation of kidney disease. Isosthenuria was present on urinalysis. Abnormalities on CBC and serum biochemistry included moderate anemia and severely elevated creatinine. Blood pressure was elevated (220/150).

Sonographic Differential Diagnosis

End-stage polycystic kidney disease with pyelectasia. Hepatic cystic degeneration.

Image Interpretation

The kidneys in this patient presented severe polycystic changes and enlargement. Very little functioning parenchyma was noted. The kidneys measured 6.3 cm. Pyelectasia was noted. There was debris in the urinary bladder both dependent and suspended. There was concomitant cystic degeneration of the liver.

DX

Polycystic kidney disease, end stage with pyelectasia.

Comments

Antihypertensives, 72 hours of IV fluid protocol, and empirical treatment for urinary tract infection would all be warranted in this patient.

Clinical Differential Diagnosis

Primary renal pathology – chronic kidney disease, pyelonephritis, polycystic kidney disease, neoplasia (lymphoma). Secondary renal pathology due to systemic hypertension.

Sampling

None. Urine culture and sensitivity would be warranted via cystocentesis.
