Ultrasound of the left and right carpal regions –
Left focal symmetrical swelling at up to 1.7 cm with decreased echogenicity, irregularly increased echogenicity and mild loss of the regular fibre pattern of the left distal flexor carpi ulnaris tendon is noted next to its insertion at the accessory carpal bone. Focal mineralization is seen within the tendon approximately 1 cm proximal to the insertion. The bone surface of the accessory carpal bone is uneven and presents moderate new bone formation.
Chronic repetitive strains usually are the underlying cause. Consider proactive conservative management with controlled load, physical and laser or shock wave therapy. Tendon healing and reconstitution of tendon fibres should be monitored ultrasonographically (recheck in 4-6 weeks). The right ulnar carpal flexor is considered within normal limits at this point.
