ORTHO – Chronic degenerative joint disease (DJD) with effusion and osteophytosis of the stifle in a 4 year old English Bulldog

This 4 year old English Bulldog presented with a sore left stifle. 

Physical Exam: sore left stifle, ambulating normally, minimal drawer

Image Interpretation

Ultrasonographic findings
Left stifle joint:
The infrapatellar recess of the left stifle joint contains a mild amount of anechoic
effusion. The joint capsule reveals moderate thickening with moderate synovial
proliferation.
The cartilage layer as appreciated onto the femoral condyles is even in thickness and
smooth in. The subchondral bone of the femoral condyles as appreciated is even and
smooth.
A moderate osteophyte formation is noted at the patellar apex and a mild amount of
osteophytes is seen at the proximal tibia.
The cranial cruciate ligament (CCL) is continuous, smooth in outline and even width.
Mild periligamentous effusion is present.
The infrapatellar fat pad is increased in echogenicity.
The medial meniscus is not seen.

DX

The left stifle joint is affected by a generalized, mild to moderate chronic degenerative joint disease with effusion, osteophytosis, synovial proliferation and capsular thickenin

Outcome

Currently there are no signs of biomechanical failure of the CCL.
Consider other underlying pathology such as patellar luxation/subluxation, immune
mediated arthropathy or infectious arthropathy. Aspiration of synovia may be helpful
for further definition.
