A 9-year-old MN Silky terrier mix dog was presented for evaluation of hematuria. Urinalysis showed inappropriate SG (1.014), hematuria, and transitional epithelial cells.

A 9-year-old MN Silky terrier mix dog was presented for evaluation of hematuria. Urinalysis showed inappropriate SG (1.014), hematuria, and transitional epithelial cells.

Sonographic Differential Diagnosis

Obstructive prostatic mass, mineralizing. These images are strongly suggestive for carcinoma. The prostatic mass obstructed the left ureter. Potential concurrent urinary tract infection.

Image Interpretation

The urinary bladder itself appeared unremarkable. The prostate measured 2.65 x 2.22 cm. The prostate was strongly mineralizing and irregular with pericapsular inflammatory pattern. This obliterated the prostatic urethra. The prostate appears to be obstructing the left ureter. The left ureter at the level of the trigone measured approximately 0.25 cm.
The left kidney presented slight pyelectasia with echogenic debris and measured 0.41 cm. The right kidney measured 4.24 cm with slight pyelectasia that measured 0.29 cm. Chronic outflow delay is likely the issue in this patient given the prostatic obstructive pathology.

DX

Mineralizing prostatic mass suggestive of carcinoma.

Outcome

None

Comments

Urethral and ureteral stent placement would be ideal in this patient. Recommend initiating Deramaxx and Enrofloxacin combination as an empirical measure.

Clinical Differential Diagnosis

Bladder pathology (chronic cystitis, neoplasia, uroliths), prostatic disease (neoplasia, abscess), renal disease (renolith, pyelonephritis, neoplasia).
