A 17-year-old SF DSH with hypothyroidism was presented for evaluation of leukocytosis, hypoalbuminemia, and elevated ALP and ALT activity.

DX

Pancreatic and duodenal mass with post-hepatic obstruction

Sonographic Differential Diagnosis

Pancreatic and duodenal mass with post hepatic obstruction. Chronic cholecystitis.
Concurrent splenic mass.

Image Interpretation

A mass in the right cranial abdomen that involved the upper duodenum and pancreas was noted and measured 3.87 x 2.49 cm. The mass obstructed the common bile duct. Ill defined, hyperechoic, surrounding fat was noted. Free fluid was noted in the region. Regional lymphadenopathy was also noted. Dilated pancreatic duct was noted owing to post hepatic obstruction and pancreatic duct obstruction.
The spleen in this patient presented multiple nodules with expansive contour and disruption of architecture. This is consistent with an aggressive neoplastic process. The primary nodule measured 0.64 cm with other smaller nodules.

Outcome

None

Comments

FNA of the pancreatic/duodenal mass is recommended to assess for any possibility of chemoreduction. FNA of the spleen would also be recommended. Poor prognosis.

Pancreatic pathology – chronic pancreatitis, neoplasia; splenic pathology – neoplasia
