Cystic mass in sublumbar region of a 6 year old FS mixed breed dog

A 6-year-old mixed SF mixed breed dog was presented for evaluation of a bloated abdomen and urinary incontinence. The clinical signs had not improved on antibiotics. Urinalysis showed 1+ bilirubin and a trace of ketones.

Abdominal mass

Sonographic Differential Diagnosis

Large, complex, parenchymal and cystic mass extending from sublumbar area to the left cranial abdomen.
Scant free fluid. No evident cardiac metastatic lesions were noted. Sarcoma is suspected.

Image Interpretation

A complex cystic mass was present with an expansive parenchymal portion of the mass that appeared to derive from the sublumbar region dorsocranial to the urinary bladder. The bladder did not appear to be involved. Free fluid and an inflammatory pattern was noted around the mass itself. It may involve a portion of the body wall. A large, cystic component and variable areas of mineralization or fibrosis were noted.
Rapid view of the heart revealed no evidence of pericardial effusion.

None

Comments

Three view chest radiographs are recommended followed by exploratory surgery and direct visualization of the surrounding omentum for potential metastatic disease that was not visible on ultrasound.

Clinical Differential Diagnosis

Incontinence – urethral sphincter mechanism incompetence, neoplasia, uroliths, extra-vesicular pressure (neoplasia, granuloma, abscess, lymph node). Bloated abdomen – fluid, organomegaly, mass (neoplasia, granuloma).
