Addison`s disease in a 10 year old spayed female Wheaton terrier

A 10-year-old SF Wheaton terrier was presented for evaluation of passing out. Physical examination was within normal limits. Significant abnormalities on CBC and serum biochemistry were regenerative anemia, mild hypoproteinemia, hyperkalemia, and a Na:K ratio of 25:1. Total T4 was within reference range, and survey radiographs were within normal limits.

Sonographic Differential Diagnosis

Screening ACTH stimulation for Addison’s disease was recommended due to the subnormally sized adrenal glands. Otherwise, there was no structural disease present.

Image Interpretation

Both adrenal glands were flattened in contour and isoechoic to surrounding tissue. The left adrenal gland measured 1.4 x 0.21 cm. The right adrenal gland measured 1.3 x 0.3 cm. Both adrenal glands were subnormal in width.

DX

Hypoadrenocorticism

Outcome

The patient was started on Percorten.

Clinical Differential Diagnosis

Addison’s disease, hypoglycemia, intermittent heart block, intra-cranial neoplasia.

Sampling

ACTH revealed hypoadrenocorticism.
