09-00158 Napoleon R Severe prostatitis, abscessation, possible carcinoma —NEED SL—-

A 10-year-old intact male Komodor with a history of bleeding from the penis was presented recumbent and dyspneic. Abnormalities on CBC and serum biochemistry were leukocytosis, band neutrophilia, monocytosis, azotemia, mild hyperglobulinemia, mildly elevated liver enzyme activity and hyperbilirubinemia.

 

Sonographic Differential Diagnosis

Severe prostatitis with abscessation. Fine needle aspirates and drainage were performed without complication. Potential for carcinoma.
Bladder stones.
Splenic thrombus.

Image Interpretation

Prostate was severely enlarged at 7.21 cm with areas of abscessation. Nodular changes noted throughout the prostate where cystic regions were drained by ultrasound guided drainage procedure. Testicles were imaged and found to be uniform. No evident pathology other than minor scrotal fluid accumulation.
Echogenic, gravitationally dependant, shadowing material in the urinary bladder.
Splenic thrombus noted in this patient. The spleen itself was unremarkable other than the splenic thrombosis. This is likely owing to hypercoagulable state deriving from chronic disease.
