09-00070 Snoopy C Bilateral Leydig cell tumors—–NEED CDX—-

The patient is a canine Beagle Cross, intact male, 11 years. The patient has a history of a pendulous abdomen, elevated ALKP, amylase and lipase. The patient has a palpable testicular mass.

DX

Leydig cell tumors were found on histopathology.

Sonographic Differential Diagnosis

Bilateral testicular tumors.
Prominent prostate.
Bilateral adrenal gland enlargement.
It is recommended to neuter this patient along with diet change. If any PU/PD is present then ACTH stimulation and blood pressure would be warranted. Full adrenal gland panel from the University of Tennessee would be ideal.

Image Interpretation

The urinary bladder, trigone and pelvic urethra presented normal wall thicknesses with anechoic urine and normal tone. No uroliths or sediment were visualized. No evidence of inflammatory or neoplastic changes were noted. The ureters were not visible and considered normal. The testicles in this patient presented bilaterally mixed hyperechoic and hypoechoic changes and no overt tumor in the right testicle with small tumors in the left testicle. There was no evidence of abdominal metastatic disease. The prostate was uniformly prominent at 3.86 cm with minor micronodular changes. Low grade prostatitis is likely. Bilateral adrenal gland enlargement.

Outcome

The patient was neutered and ACTH stimulation was performed. Results revealed findings consistent with Cushing’s.
