A 2-year-old intact female miniature Poodle was presented for evaluation of vomiting for ten days and lethargy. Abnormalities on CBC were neutrophilia, monocytosis, and thrombocytosis. Survey radiographs showed gastric deviation, excessive gastrointestinal gas accumulation and a mass effect in the cranial abdomen.

 

Sonographic Differential Diagnosis

Gastrointestinal string foreign body. Concurrent live pregnancy with some likely resorbing fetuses; at least two live fetuses. Exploratory surgery is essential in this case for the gastrointestinal foreign body and whether the pregnancy is to be maintained or terminated would be a judgment call by the surgeon and owner. Two live fetuses appear present; however resorbing fetuses also appear to be an issue. Concurrent metritis may be playing a role. Immediate exploratory surgery is recommended primarily for the GI tract. Expression of the gallbladder is recommended at surgery with liver biopsy given the hepatic remodeling.

Image Interpretation

Distal small intestine revealed linear foreign body with dilated bowel and obstructive pattern. Shadowing foreign body noted in the stomach with dilated obstructive pattern. Linear foreign body with accordion pleating. Regional inflammation noted. Trace amounts of free fluid followed by empty small intestine.
Dilated uterus was also noted with doubled layered wall. Two separate live fetuses appeared present; however, echogenic wall thickening noted with the uterus as well.
