Lots of attached mucous

Lots of attached mucous debris and wall thickening and some sand. Needs cystolomy and wall bx and culture and lavage. Often these are lympholpasmacytic cystitis cases if not neutrophilic or vasculitis based. If LP then pred management may be in order but I wouldnt tx pred without the histopath to support it. Culture the sand or wall as well. This is an ongoing study of ours so please submit histopath if you have it. Nice post

With that degree of anemia and hematuria, rule out a hemorrhagic diathesis before biopsy.

Thank you very much for the comments. The bleeding slowed, urine became clearer. hematocrit was at 23% the next day. Unfortunately the owner has no more funds, so no surgery etc; we discharged the cat yesterday and I will be rechecking him this evening – hoping he can pull himself through this, seems like it’s up to him at this point…..

Just an FYI ive tracked these

Just an FYI ive tracked these cases where there are shadowing stones one day, no stone and minor debris 5 days later with med tx alone, and then stones back again in 7 days with same continual therapy….so its important if going in on these cases to scan again right before sx and inform the owner of extpected findings but always get histopath and inflammatory cell type predominance in the wall.
