Abnormal Liver

– 10 year MN Australian Shep presented for routine physical exam

– mild weight loss noted and my asscociate thought she could feel a mass or enlargement in the cranial abodmen; otherwise no other clinical symptoms

– bloodwork is pending

– no masses seen on ultrasound however the liver is small and the capsule nodular; the left liver lobes looks really unusual – kind of shrivelled up?

Could this be a CAH liver with early fibrosis, cirrhosis? Nodular hyperplasia?

Looks like a combination of

Looks like a combination of hepatic remodeling and nod hyperplasia or potential HCC needs bile acids and bx both the nodule and the general parenchyma. Maybe power doppler th enodule to check vascuylariuzation… lipogranulomas and benign things in general dont light up readily and this does not seem to loose too much architecture which is good.

The cbc, biochem ended up

The cbc, biochem ended up normal in this patient but I guess bile acids are still warranted as you can have liver dysfunction without liver enzyme abnormalities?

Liver enzymes only useful for

Liver enzymes only useful for acute damage. In chronic diseases susch as cirrhosis, liver enzymes are normal as there is no ongoing leakage from damaged hepatocyes. With normal albumin and normal MCV unlikely to have significant liver disease but still need bile acids and a biopsy to fully evaluate the liver.
