3 yr FS DSH with persistent urinary incontinence. Urinalysis x 3 reported hematuria, mild number of WBCs, no bacteria or crystals and good concentration. One urine culture performed was negative. Pet has been on Clavamox and Zenequin previously. The only medication that seems to make a significant difference is gabapentin but owner is concerned about long term safety. Suspect this is idiopathic cystitis, but please comment on the urinary bladder. Are these just crystals with some shadowing vs. very small uroliths vs. gas?

There is both floating and dependent hyperechogenic sediment – most likely dealing with idiopathic/interstitial cystitis.

If not already on put onto a urinary-type diet such as Hills CD-stress. Gabapentin is controlling the pain associated with the cystitis and try and get the dose to the minimum effective dose. Environmental enrichment can also help in these cases.
