Poor urethral tone in an 8 year old FS mixed breed dog with hematuria

An 8-year-old female spayed mixed breed dog was presented for evaluation of hematuria. Urinalysis showed pyuria, hematuria, and low-normal specific gravity (1.026).

Sonographic Differential Diagnosis

These images show a large amount of urinary bladder sand and suspended bladder debris, with poor urethral tone. Underlying occult incontinence, contributing to recurrent urinary tract infection, is likely. The sand probably consists of struvite crystals.

Image Interpretation

The urinary bladder contained suspended debris, with minor apical wall thickening and sand accumulation measuring approximately 3.0 cm. The pelvic urethra was significantly dilated with poor tone and measured 1.0 cm in width with uniform wall to a position of approximately 3.0 cm distal from the cystourethral junction.

DX

Urinary bladder sand. Suspended debris. Poor urethral tone.

Outcome

A repeat ultrasound was performed 4 days later and revealed a bladder containing accumulation of sand and small calculi that measured approximately 4.0 cm. The apical ventral wall of the bladder was still thickened with suspended debris.

Comments

Cystotomy, sand analysis and culture would be recommended. Apical wall biopsy and culture would be ideal, followed by 4-6 weeks of antibiotic therapy.

Clinical Differential Diagnosis

Bladder disease: bacterial cystitis, urolith, neoplasia, polyploid cystitis; Renal disease: renolith, pyelonephritis, neoplasia.
