Decreased corticomedullary definiton in a 2 yr old Shih-Tzu

  • 2 year old mn asymptomatic Shih-Tzu
  • Abdominal US performed because of a slowly rising ALT=166 IU/L
  • Kidneys show increased cortical thickness, increased medullary echogenicity and decreased corticomedullary definition
  • Urinalysis is normal with a USG=1.047 and negative for proteinuria
  • The liver and all other organs are essentially normal.
  • Why do the kidneys look like this?  Could this be renal dysplasia?  Should I not be concerned since kidney function is normal at this time?
    • 2 year old mn asymptomatic Shih-Tzu

    • Why do the kidneys look like this?  Could this be renal dysplasia?  Should I not be concerned since kidney function is normal at this time?
    • I am recommending a hypoallergenic diet trial and Denamarrin for the rising ALT with a recheck of the ALT in 6-8 weeks.

yeh shihtzus, Maltese and any

yeh shihtzus, Maltese and any breed a grandma would have to watch Opra with will do this:) I have yet to have any of the renal gods explain it to me but I think of it as a breed variant if normal size and values doing well. Anyone else have input on this? I would love to put a core bx needle in it just to find out but tough sell.

Ultrasound not really typical

Ultrasound not really typical for dysplasia and with a normal SG and no azotemia makes dysplasia very unlikley. Any possiblity that the dog was dehydated at the time of the ultrasound?

The dog is clinically normal but may have been fasted for the scan.  I will find out.  Does dehydration cause these changes in renal echogenicity and corticomedullary definition?

With dehydration there would be less fluid within the renal tubules/collecting ducts and thus the kidney may appear more hyperechoic. It has been reported that dehydated babies show hyperechoic renal medullary pyramids with speckled foci suggestive of crystal deposition that reverse completely with fluid therapy.
