liver mass left lobe?

liver mass left lobe?

– 15 year MN DSH with history of slight weight loss and decreased appetite

– bloodwork elevated calcium (likely not fasted and not ionized calcium test) with everything else wnl

– multiple large cysts, some septated in the right liver

– large cystic-mass filling much of the left cranial abdomen also present – it appears to be coming off the very tip of one of the left liver lobes

– spleen, both kidneys and pancreas and rest of exam normal except for a trace abdominal effusion – I could not attach this mass to anything else

– I have recommended exploratory laparotomy as the left (liver lobe?) mass looks like it can be removed and is likely causing some abdominal discomfort

Does this mass appear to be attached to the liver ? Is this a biliary cystadenoma? other?

(colour Doppler didn’t help me out much here )

This is either a primary

 
 

This is either a primary liver cyst/manifestation of polycystic liver disease or a cystadenoma maybe adenoca but doubt it. The peril here is the pedunculated aspect of the left lateral mass as its predisposed ot torsion so removing that one would be indicated.

You can tell this is part of the liver because the mass parenchyma is contiguous with the normal liver and portal walls run right into the mass without being separated by a capsule… follow the lines of the infrastructure of organs and they will take you to answer your own question:)

This example is an older case and lacks video but a similar lesion that walked into the door bleeding out as the torsed mass was hanging on by a thread…very lucky cat.. I know I have video on this somewhere when I find it I will add it.

Thanks Eric – good to know about the potential for torsion. I didn’t even think about that. Will see if we can expediate surgery on this one.
