Severe urocystitis in an 8 year old MN DSH cat

An 8 year old neutered male DSH cat was presented for not urinating. Urinalysis revealed proteinuria, hematuria, and triple phosphate crystaluria. Urine culture was negative for bacterial growth. The only abnormality on blood work was mild hyperglycemia.

Sonographic Differential Diagnosis

Emerging perforation at the bladder apex. Likely interstitial cystitis, possible urachocele.
Minimal possibility of bladder lymphoma.
Urinary bladder sand.

Image Interpretation

The urinary bladder presented a 2.0 cm of accumulated sand with a 1.5 x 1.0 cm apical wall thickening that demonstrated a hypoechoic, mural change. This image is consistent with a hematoma. Minor omental reactivity was noted at the apex, which is predisposed to perforation. A resection of this area of the bladder would be recommended at the time of cystotomy. The proximal urethra does not show evidence of obstruction.
