Splenic mass in a 7 year old MN Vizsla

A 7-year-old MN Vizsla was presented for diarrhea and vomiting for three days. Physical examination showed pyrexia (103.3°) and a soft non-painful abdomen. CBC and blood chemistry were within normal limits. Radiographs showed a round, mid-abdomen mass, possibly splenic. The patient was treated with subcutaneous fluids, Centrine, ampicillin, and Cerenia.

DX

Round cell neoplasia suspected

Sonographic Differential Diagnosis

Large vascular splenic mass, not typical of hemangiosarcoma. Round cell neoplasia was suspected. This appeared respectable.

Image Interpretation

The abdomen in this patient presented a 5 x 4.6 cm mid splenic mass. This is highly vascular and parenchymal. No visible metastatic lesions were noted. There was no evidence of rupture noted.

Outcome

An exploratory surgery was recommended with direct visualization for any metastatic lesions in the liver. Three-view thoracic radiographs and an echocardiogram were also advised. The patient remained overnight for observation and was discharged the following day with Flagyl, amoxicillin, and I/D diet. At last communication the owner had decided not to pursue surgery.

Clinical Differential Diagnosis

Vomiting – gastrointestinal disease (viral/bacterial gastroenteritis, IBD, foreign body, partial obstruction, neoplasia, perforation with focal peritonitis). Mass – neoplasia, abscess, granuloma, torsion, or infarction (originating in the spleen, liver, kidney, pancreas, lymph node).
