Perforating GB?

– 12 yr old FS Pomerarian

– vomting, anorexia, lethargy

– increased liver enymes, WBC

– ultrasound shows hyperechoic reactive tissue around the periphery of the GB

– positive Murphy sign

– I’m pretty sure this is a perforating GB mucocele but not a classic kiwi-fruit presentation?

– I have recommended that this pet be taken immediately to surgery for cholecystectomy and liver biopsy

Sound like the right plan?

Yep classic inflamed doubt if perforated yet though as the ill-defined fat is localized to the nexk. Suspened ice cream swirl bile in the GB. I bet you got a + murphy sign when scanning the Gb neck???

To cut is to cure here… sit on it and may perf and dead dog 🙁 Once clinical (ADR) and inflamed I cut them for sure. Once a bad gland always a bad gland.
