High velocity tricuspid inflow on 17 yr old cat

      • 17 yr old mn DSH presented for predental anesthetic echocardiogram
      • PE showed 2/6 systolic murmur and 4/4 dental disease, other hx includes loose stool and vomiting controlled with prednisolone, B12 injections, probiotics, and RX diet,  1/4 early kidney disease, proteinuria, and arthritis.
      • Echocardiogram shows mild conentric LV thickening (IVS, LVPW=6.0mm).  Doppler studies performed just above the tricuspid valve show turbulent blood flow with peak measured velocity of 2.6m/s.  No VSD’s seen on RPLVOV and LPLVOT views and really could not prove it to myself on left transverse view either.
      • This cat is 17 years old, so just wondering what the significance of this higher vel tricuspid valve flow is?  Tricuspid stenosis?  Very small VSD? DRVOTO?

Hi!

The flow pattern would mach best some dynamic intraventricular obstruction. If it was a tricuspid stenosis, I would expect a large right atrium which does not seem to be the case here. I can’t rule out a small VSD based on these images/clips but then the acceleration time of the spectral profile would be very very short which is not the case here.

Summarizing, based on these images, I would not worry too much about this old kitty.
