This looks like an EHPSS
This looks like an EHPSS splenocaval. Should be amenable surgically if the liver can handle the new portal pressures.

SDEP Abdomen unleashed has the shunt hunt maneuvers in it as well but you imaged this one nicely as its a short shunt connecting the splenic vein bypass of the portal vein entry and goes directly into the cvc… but splenocavals are often missed because they are short and th epatient doesnt want the probe there. But actually these are the most common (+/-65% of all ehpss are splenocaval).
